Banner University of Arizona Health Plans is currently seeking a Transitional Care Navigator to join the Case Management Department!
The Transitional Care Navigator provides critical support to the Case Management Department which provides multifaceted healthcare navigation assistance to members/caregivers and providers in accordance with regulatory compliance standards. The position performs a variety of functions that support the licensed staff to meet regulatory requirements.
Preferred qualifications include:
Demonstrated experience in coordinating specific complex on-going and one time projects which may require contacting providers and members regarding compliance requirements
Demonstrated ten key experience
Experience with computers, data entry, MS Word and MS Excel
Excellent written and verbal communication skills, and building relationships
Experience in healthcare as a Nursing Asst, Medical Asst, Health Unit Coordinator, Patient Care Tech
Monday- Friday; business hours 8:00AM-5:00PM

This position supports the smooth, timely, and coordinated client transition from acute care to alternative levels of care including but not limited to post-acute settings, community services, or physician offices as directed by the care coordination team. This position performs follow-up tasks identified in the inpatient discharge care plan for management of Banner patients across the healthcare continuum.
