Licensed Physical Therapist and able to practice in the State of Arizona. Able to perform all duties and responsibilities as a physical therapist in an inpatient and/or outpatient setting. Able to adequately supervise support staff.
Essential Functions / Major Responsibilities:
Perform patient evaluations, formulate plans, provide direct patient care and supervise Physical Therapy Aides and Physical Therapy Assistants.
Appropriately document patient progress on a daily basis.
Appropriately document patient evaluations and progress for all patients.
Send initial evaluations and progress reports in a timely fashion.
Complete all duties of a Physical Therapy Aide and Physical Therapy Assistant.
Attend in-service and continuing education.
Attend other meetings per choice of department director.
Help in implementation of policies and procedures.
Address pediatric, adolescent, adult and geriatric patient and family concerns, demonstrating specific knowledge of the needs, development and care and/or treatment of the pediatric, adolescent, adult and geriatric patient population.
Complete continuing competence for 2 year renewal period.
Maintain current CPR certification and TB test. Must have within 30 days upon hire and renewal.
Notify Board with name and address change within 30 days.
Displays proper etiquette and mannerisms that reflect the Keys to Excellence.
Promotes the Patient Safety Standards as a core value of the organization.
Education and/or Experience:
Graduated of an accredited Physical Therapy program (required).
Licensed in the state of Arizona (required).
High School diploma or equivalent (required).
Current CPR/BLS certification (required).
WILL CONSIDER NEW GRADS ( 1 + Years experience preferred)
