The Banner Boswell Skilled Nursing Facility is a 91-bed facility affiliated with the Banner Health System has an overall rating of 4 out of 5 and has a short-term rehabilitation rating of High Performing! We are not for profit facility. Our Nursing Patient ratios are 1:10 which is very generous in the SNF environment. We pride ourselves on our excellent Therapy Team and Nursing Care who provide great patient care and assist our patients in getting to their next transition in care. We are proud of our length of stay which discharges patients to their next transition in care in just the right time where they can continue their recovery in the comforts of a more homelike environment.
We seek an energetic and enthusiastic self-starter to join our team as an Activities Coordinator. The ideal candidate will be a motivated professional, with a passion for making a difference in the lives of seniors. You will play a vital role in our patients well-being and recovery by developing and planning engaging activities for the mind and body, and promoting social interaction for all our patients.
Qualified candidates must be a registered Therapeutic Recreation Specialist or Activities Professional, or a qualified Occupational Therapist or OTA, and is eligible for certification as therapeutic recreation specialist or activities professional by a recognized national accrediting body. Min of 2 years of experience strongly preferred.

Help patients return to as independent a lifestyle as possible at the Skilled Nursing Facility at Banner Boswell Rehabilitation Center. This short-term, inpatient rehabilitation facility is designed to maximize each patient's functional abilities. Some of the most common care pathways at the skilled facility include recovery from hip fractures, stroke, diabetes, joint replacement and debility/weakness.
POSITION SUMMARY This position assesses the leisure interests and needs of for individuals in long-term care settings, conducts activity programs, and transports residents to off-site activities to promote residents' physical, social, mental, and intellectual well-being and independence as governed by state, federal, and accrediting regulatory agencies.
CORE FUNCTIONS 1. Assesses leisure interests, abilities, and physical needs of residents, and creates activity plans appropriate to age and ability to promote enhanced quality of life.
2. Plans and conducts individual and group activity sessions and special events.
3. Documents resident participation and advancement according to professional and regulatory standards. Reviews activity plans and adjusts activities as needed to accommodate resident changes.
4. May be required to transport residents to off-site events.
5. Works independently under general supervision. Follows and creates work procedures. Analyzes needs and determines own work routines to meet needs. Confers with supervisor on any unusual situations. Internal customers: All levels of skilled nursing management and staff, medical staff, and all other members of the interdisciplinary health care team. External Customers: Physician offices and resident families.
MINIMUM QUALIFICATIONS This position requires a professional who is a qualified therapeutic recreation specialist or an activities professional who--
(i) Is licensed or registered, if applicable, by the State in which practicing; and
(A) Eligible for certification as a therapeutic recreation specialist or as an activities professional by a recognized accrediting body on or after October 1, 1990; or
(B) Has 2 years of experience in a social or recreational program within the last 5 years, one of which was full-time in a therapeutic activities program; or
(C) Is a qualified occupational therapist or occupational therapy assistant; or
(D) Has completed a training course approved by the State.
Min High school diploma/GED or equivalent working knowledge. Cardiopulmonary resuscitation (CPR) certification.
Must posses a valid driver's license and be eligible for coverage under the company auto insurance policy.
Employees working at the Boswell Skilled Nursing Facility must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment.
Two years experience with activity program. Requires the ability to work collaboratively with physicians and other healthcare providers. Must have strong oral, written, and interpersonal communications and human relation skills.
Experience with dementia. Additional related education and/or experience preferred.
