Gastroenterology (GI) is a specialty practice area in which health-care staff provide care to patients with known or suspected gastrointestinal problems who are undergoing diagnostic or therapeutic treatment or procedures. The Medical Instrument Technician Gastroenterology (MIT/GI) assists the practitioner in endoscopic procedures that are routine and standardized in nature including, but not limited to; esophagogastroduodenoscopy (EGD), colonoscopy, and bronchoscopy. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. There are no specific educational requirements for this occupation. Education may be substituted for experience only at the GS-4 and GS-5 levels. See the grade requirements part of this standard for information regarding educational substitutions. Licensure or Certification. Licensure or Certification is not required for this occupation; however, it is strongly desirable at GS-6 or above; desired certification: Basic Cardiac Life Support (BCLS) Advanced Cardiac Life Support (ACLS) certifications are desirable. English Language Proficiency. All Medical Instrument Technicians must be proficient in spoken and written English in accordance with VA Handbook 5005, chapter 2, section D, paragraph 5a, in order to perform the duties in this position. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Medical Instrument Technician (GI) GS-5 (a) Experience. At least 1 year of experience comparable to the next lower grade level which demonstrates the knowledge, skills, abilities, and other characteristics related to the duties of the position to be filled. This would be experience in operating equipment related to the particular specialization for which application is made or in related functional areas. Experience gained in the operation of equipment for animal diagnosis or treatment may be credited through this grade level. In addition, the candidate must demonstrate the following KSAs: OR, (b) Education. Successful completion of [4 academic years above high school leading to a bachelor's degree with courses related to the occupation, or a bachelor's degree in a major field of study appropriate to medical instrument technician functions. (c) Demonstrated Knowledge, Skills, and Abilities 1. Knowledge of standard medical terminology to interpret physician orders or instructions. 2. Knowledge of common physical disabilities and ability to position patients for the examination or treatment. 3. Learn the normal and abnormal results for routine procedures to recognize and report obvious abnormalities. 4. Operate the equipment to administer routine/standard diagnostic treatment or procedures. 5. Perform standard operator maintenance on the equipment including the ability to disassemble, clean, reassemble, and calibrate the machine. Medical Instrument Technician (GI) GS-6 (a) Experience. At least 1 year of experience comparable to the next lower grade level which demonstrates the knowledge, skills, and abilities related to the duties of the position to be filled. In addition, the candidate must demonstrate the following KSAs: (b) Demonstrated Knowledge, Skills and Abilities 1. Ability to set up endoscopy equipment, ensuring endoscopes and ancillary equipment functions properly. 2. Ability to aid the physician with standard and routine procedures such as colonoscopy and esophagogastroduodenoscopy (EGD). 3. Knowledge of medical terminology and human anatomy in order to assist practitioner with routine standardized gastroenterology procedures. 4. Ability to interact with patients explaining procedures and positioning the patient for the procedure, recognizing disabilities or impaired mobility and assisting the patient as necessary. 5. Ability to reprocess and maintain specialty critical reusable medical equipment (RME), following approved Standard Operating Procedures (SOPs) and universal precautions. 6. Ability to use basic computer systems and programs to record patient and endoscopic data, adhering to patient privacy and information security policies. 7. Ability to manually clean and disinfect non-critical reusable equipment and surfaces, dispose of one time-use equipment and trash per facility policy, and restock supplies such as linens, suction equipment, and cleaning products. (c) Certification. No certification is required; however, it is desirable that employees at this level have Basic Life Support (BLS) certification. Medical Instrument Technician (GI) GS-7 (Full Performance Level) (a) Experience. At least 1 year of experience comparable to the next lower grade level which demonstrates the knowledge, skills, and abilities related to the duties of the position to be filled. This would be experience which provided knowledge of proper performance of endoscopic equipment and procedures. In addition, the candidate must demonstrate the following KSAs: (b) Demonstrated Knowledge, Skills, and Abilities 1. Knowledge of endoscopic equipment and use in basic and complex endoscopic procedures. 2. Knowledge of anatomy and physiology related to the GI system, with ability to recognize and report obvious abnormalities during procedures. 3. Ability to assist physician with specialized, complex procedures. 4. Knowledge of universal precautions and disinfection techniques and ability to follow SOP's to clean reusable medical equipment (RME). 5. Ability to collaborate, communicate, and demonstrate customer service skills and interpersonal skills with all healthcare professionals to ensure quality and continuity of care. 6. Ability to use basic computer systems and programs to record patient and endoscopic data, adhering to patient privacy and information security policies. 7. Ability to deliver patient care, including vital sign measurements and medication administration. (c) Certification. No certification is required; however, it is desirable that employees at this level have BLS certification. It may involve walking, frequent bending, reaching, and stretching to set up and take apart equipment; lifting and positioning patients; and carrying, pushing, or pulling, moderately heavy objects. Duties may require above average dexterity. The full performance level of this vacancy is GS-7. Medical Instrument Technician Gastroenterology (MIT/GI) GS-05 is considered a trainee and at this level operates and monitors commonly used equipment performing routine procedures under normal supervision. The MIT/GI trainee technician functions somewhat independently in carrying out standardized procedures of limited complexity. The GS-5 Medical Instrument Technician position is established to provide training in technical and clinical skills for diagnostic procedures performed in the Endoscopy rooms within the Medical Center. This position is developmental to Medical Instrument Technician GS-649-6 and targeted to Medical Instrument Technician GS-649-7. Performs as a responsible member of the GI Department team and interacts with patients, family members, professional and supportive personnel who provide patient care, and with members of other services, incorporating acceptable, established customer service standards into practice. Responsible for the procedure room set-up and tear-down, specimen collection and assisting the doctors and nurses during endoscopy procedures. Room set-up involves choosing appropriate scope and accessories for the upcoming case, assuring they are properly connected to processors, etc. Stocking the room so all needed supplies are at hand and the case runs smoothly. During cases the technician will be available and trained to assist with accessories and endoscopic treatment and interventions. After the case and the patient has been moved to recovery, the GI Technician will assure the appropriate initial pre-cleaning, cleaning, high level disinfection, and storage of endoscopes and accessory equipment. The MIT (GI) prepares the endoscopy suite for the next procedure, restocking as needed. Collects, processes, and labels all specimens; train other technicians in basic and advanced endoscopic procedures; works in and outside the GI unit offering technician support during emergent and on-call endoscopic cases; The MIT/GI assures procedure reports are completed and documented in CPRS as soon as possible after interpretation. Inventory and maintain par levels of specialty equipment for the endoscopy unit. The Technician will reprocess and maintain specialty critical reusable medical equipment (RME), following approved Standard Operating Procedures and universal precautions. Coordinates and carry out day-to-day operations of the GI laboratory and procedures and other tasks related to the efficiency of the unit. The GI Tech performs, reports, and records assigned duties, including but not limited to: Assist physicians directly with diagnostic and therapeutic procedures (EGD, ERCPs, Colonoscopy, EUS, EBUS, Manometry, PH, Capsule Endoscopy, Fibro Scan, PEG placement) including setting up equipment, assisting with biopsy procurement, snare polypectomy, esophageal/pyloric/colonic dilation, variceal banding, and/or electro cautery for treatment of bleeding. Provides technical support and assist with post-procedure equipment care. Has the skill and competency for the handling, cleaning, high-level disinfection, and storage all endoscopic scopes and department equipment. Serves as a liaison to hospital biomedical equipment repair section as well as outside repair vendors in the absence of the Lead Technician. Responsible and knowledgeable for imaging equipment and the use of the ProVation imaging system used during the procedures and assists with updating schedules daily for all procedures. Prepares patients for procedures and room turnover activities, including proper patient position during the procedure and attaching monitoring devices. Helps with transferring and transporting patients to recover/discharge after procedures. Collaborates with RN to ensure QI/QM measures are met. Is competent to assist the GI Providers outside of the GI department, in the OR, ICU, or ER with all GI procedures. Attends in service training and education and assists other hospital personnel regarding the cleaning, maintenance, safety, proper operation, and troubleshooting of endoscopic equipment and accessories; maintains associated documentation as appropriate. Assists the RN or MD in addressing patient's needs for treatments, accurately administering treatment medication as prescribed according to established polices/procedures as directly ordered by the GI Provider during the procedure.
