The Patient Navigator assumes responsibility and accountability for the management of resources through interdisciplinary collaboration to achieve optimal patient outcomes. The Navigator coordinates research data, community physicians, hospital resources into a seamless model of access and care benefiting patients, participating physicians, and family members. The Patient Navigator will coordinate the patient's care to increase the efficiency and quality of care that is delivered to the patient, from preventative scans through the patient's journey throughout their care.
